Short version. We collect what you tell us on our form so we can contact you about therapy. We do not sell your information. We never share your phone number or SMS consent with anyone for marketing. Health information you give us is handled under HIPAA.

To contact you about the request you submitted
To verify your insurance benefits before your first visit
To schedule and deliver therapy services
To measure which advertisements bring people to us

4. What we do not do

We do not sell your personal information.
We do not rent or trade your information.
We do not share your mobile number or your SMS consent with third parties or affiliates for their marketing purposes.
We do not send your health information to advertising platforms.

5. Who we share with

We share information only with service providers who help us operate, and only as much as they need:

Our customer relationship platform, which stores your contact record and sends our messages, under a Business Associate Agreement
Our scheduling and electronic health record system
Your insurance company, for benefit verification and claims

We may also disclose information when required by law.

6. Advertising and analytics

We use the Meta pixel to measure advertising performance. It is configured to send only that a form was completed. It does not send your name, email, phone number, or anything you told us about your health. Your answers are never placed in a web address.

7. Health information

Once you become a patient, your health information is protected under HIPAA and by our Notice of Privacy Practices, which you receive during intake. Information submitted through this form before you become a patient is treated with the same care.

8. How long we keep it

Contact records are kept as long as needed to serve you and to meet our legal and professional record-keeping obligations. You may ask us to delete your information at any time by emailing us, subject to records we are required to retain.

9. Your choices

Text messages: reply STOP to any message.
Email: use the unsubscribe link in any email.
Phone: tell us and we will stop calling.
Access or deletion: email [email protected].

10. Children

This site is intended for adults. We do not knowingly collect information directly from children. Parents and guardians submit information on behalf of minor patients.

11. Changes

If we change this policy we will update the date at the top of this page.
